Community Outreach
Upon your arrival at Maple Lawn Health Center, you will be greeted by Pat Reinkensmeyer, Guest Relations / Community Liaison. Pat will show you to your room and make introductions to the Admissions Team as they take you through the admission Process. Pat will also assist you and your family in filling out the required paper work and will answer any questions you may have.
Admission Procedures
Upon your arrival at Maple Lawn Health Center, someone from the nursing or social services department will greet you and escort you to your room. Introductions to your roommate will be made if applicable. A Certified Nurse Assistant (CNA) will record your vitals, and a nurse will complete a physical assessment. Social Services will assist you

Intergenerational Sandbox
in filling out the required paperwork and will give you information including available services, privacy practices, advance directives, and resident rights and responsibilities. A tour of the facility will be offered to you. Meal arrangements will be made including special diets.

Your facility physician will visit on his or her next scheduled facility date unless otherwise indicated. A therapy evaluation will take place if your physician has ordered therapy services such as physical therapy, occupational therapy, or speech therapy. Our registered dietitian will meet with you to explain dining room procedures and discuss food preferences. Someone from the Activities Department will meet with you to determine activity interests and will inform you of our extensive activity programming. One of the Health Center chaplains will also visit to welcome you to the facility.

What to Bring

Legal documents:
Social Security card
Medicare card
Insurance card
Long term care policy
Living Will
Power of Attorney paperwork

Please remember to mark all items, including you glasses, hearing aids, and dentures.

Clothing/Belongings:
7 changes of clothing
7 pairs of pants
7 tops
7 undershirts/underpants
7 pairs of socks

Lightweight jacket or sweater
Robe (if desired)
4 pairs of pajamas
Slippers
Rubber soled shoes for therapy
* Consider limitations of movements when selecting clothing items in relation to therapy needs *
All personal items must be marked with the residents name on them. A laundry marker is provided upon admission at Maple Lawn for your convenience. Upon request, we will furnish iron on name labels for other clothing. If you iron these on, we request that you also stitch them in a couple of places.

Toiletry Items:
Electric Razor (disposable razors are used at the facility)
Comb/Brush/Pick
Denture brush/Toothbrush
Toothpaste/Denture tabs
Denture case
Lotion
Shampoo/Conditioner
* A welcome basket is provided for all new admissions including most toiletry items, however they may not be the exact brand that you prefer *

Special Items:
Please bring any special pillow or blanket that your loved one may prefer. You may also bring pictures of family members or loved ones. Feel free to make your loved ones room as comfortable as possible for their stay at our facility. We encourage family involvement, and would like our new resident to have a living space that is as comforting as possible during their stay at Maple Lawn. The items listed are just suggestions and may need to be increased depending on individual needs.

Laundry:
Residents have the choice of their laundry being done by Maple Lawn, however if a family member or friend would like to do laundry this is fine. Please let us know upon admission who will be doing laundry for the new resident. There is no extra charge for facility laundry services.


Frequently Asked Questions
Where should I go when I arrive?
Whether you arrive by ambulance or private car, just follow the signs to the Health Center. You will enter via the front door. There is a receptionist just inside the doors who will have a staff member show you to your room. If you need assistance in transferring from the car, your driver can let us know and we will gladly assist you.

Am I allowed to keep spending money in my room?
Yes, however we suggest that you open a Resident Fund account with the front office, as we do not encourage persons to keep money in their room. You can withdraw money Monday through Friday during normal business hours.
